Reminder: Click the FORMS tab after to complete your form which relates to your visit. -Late Fee : $25 after 10 Minutes -Remainder balance is due in full at the time of Service. -APPOINTMENTS CANNOT BE RESCHEDULED WITHOUT 48 HOURS ADVANCE NOTICE....SAME DAY RESCHEDULING and/or RESCHEDULING WITHOUT 48hours will Require $150 and full fee retained (or YOUR APPOINTMENT WILL BE CANCELED, RETAINER IS FORFEITED, and FUTURE APPOINTMENTS WONT BE PERMITTED) -Appointments are made online with deposit required and consent forms are required to be completed 48 hours before appointment. GRACE PERIOD IS 15 MINUTES...AFTER 15 MINUTES YOUR APPOINTMENT IS CANCELLED (NO CALL/NO SHOW CANNOT BOOK FUTURE APPOINTMENTS WITHOUT PAYING FULL SERVICE FEE UPFRONT) -CANCELLATIONS: ITS YOUR RESPONSIBILITY TO CANCEL OR RESCHEDULE YOUR APPOINTMENT 48hrs PRIOR TO SERVICE DATE.
